Click here to go to our printable application form. Complete the application form, print it out, sign it and then post or fax to your nearest Bingo. Your Players Card will then be ready for you when you next visit the Bingo, or it can be posted out to your home address.

Applying in person

Go along to your nearest Carlton Bingo, complete the application form, sign it and you will be an instant member of Carlton Bingo.

On-line application

To become a member complete the following form and click send.  An email will be sent using your email software. If you are using Outlook, a warning will come up asking if you would like to send. Please accept the send email.  Other email software will prefil an email message for you to review and send.  Please check that all information is correct before sending as incorrect information will delay your application process.  Your application will be processed by your nearest bingo.  Your new Players Card will be ready for collection when you visit the Bingo premises.  You must sign the application form before your player card is released.
